All refs who wish to be paid for their labors must have a W-9 form on file with the LGUSL Treasurer.  If you haven't turned one in, then download a blank W-9,.  Completed forms, including your signature, can be dropped at Sabine's.  You must sign the form before turning it in.

You’ve shown a red card.  Now what?  As part of your job and responsibility, you’re required to file a report within 24 hours.
